Practice Nurse – Health Assessment/Chronic Disease Management

About the Role:

Macksville Medical is looking for a caring and dedicated Practice Nurse to join our friendly team. If you’re passionate about providing excellent patient care and have experience in health assessments and chronic disease management, we’d love to hear from you!a

Key Responsibilities:

  • Support our doctors with health care assessments and chronic disease management.
  • Be the first point of contact for chronic disease management and patient health assessments.
  • Represent our practice professionally.
  • Follow our practice policies and procedures.
  • Participate in achieving practice goals.
  • Maintain high levels of infection control.
  • Engage in quality improvement activities.

What You’ll Do:

  • Create a safe and welcoming environment for patients, visitors, and staff.
  • Use standard precautions when dealing with body substances and manage sharps disposal.
  • Report any incidents or errors to the Practice Manager.
  • Provide clinical support to doctors and other clinical staff.
  • Perform thorough health assessments and management plans, keeping recall systems and bookings up-to-date.
  • Advise doctors on patient needs for further management.
  • Maintain accurate and up-to-date medical records.
  • Perform other tasks as directed by the Practice Manager or Directors.

What We’re Looking For:

  • Current registration with the Nursing and Midwifery Board of Australia.
  • Ability to maintain confidentiality.
  • Excellent communication skills and a strong customer service focus.
  • Ability to work independently and prioritize tasks.
  • Good organizational skills and attention to detail.
  • Ability to remain calm under pressure.
  • A focus on quality improvement.
  • Strong written and verbal communication skills.
  • Understanding the needs of a diverse patient population.
